These are starting structures, not finished documents. The parts that matter are the ones you change: your tier names, your prices, your response times, your exclusions. A care plan template used unedited will describe a business that is not yours.
The exclusions section is the one most agencies skip and most regret skipping. Writing down what is not included, explicitly, prevents the slow expansion of scope that turns a profitable account into an unprofitable one over eighteen months.
